6.20 or earlier:

1. Download OFW 6.20 online, put it on your memory card in PSP/GAME/UPDATE. Boot up the PSP and install it.
2. Download the CFW (6.20 in this case), use the Updater, first test, then install the CFW (You put the folders in PSP/GAME).
3. Finally, use the Permanent Patch.

6.21 or later:

1. Download OFW 6.60 online, install it.
2. Use the 6.60 PRO Updater, install the CFW.
3. By stock, the PSP will always boot to OFW, to turn CFW on, boot the Fast Recovery application which is in the package.

There is a way to make the 6.60 hack permanent, but you have to use CIPL flasher. The alternative is downgrading.
As far as I know, 6.60+ Firmwares are not hacked just yet.
